*If you are over the age of 65 and/or are eligible for Medicare Part B services, we are mandated to bill Medicare Part B directly for all services that are not “Wellness” services *
Effective January 1, 2022- We fully comply with the No Surprises Act for uninsured and private pay patients.
Home Assessments
Expert Home Evaluation & Assessment: $350
Includes: One Comprehensive In-Person Evaluation Fully Detailed “Room to Room” Report Including Recommendations for Falls Prevention & Safety.
Reports will be sent by email, within 3 business days, in a PDF format. We can mail a printed copy at your request at no additional charge.
Photos Optional - Please Discuss Prior to the Home Assessment.
At this time we are not licensed to complete the installation of the recommended safety modifications. However, we can refer you to a licensed/bonded reputable contractor in the area.

Humana (OON)
We credentialed as out-of-network (OON) providers with TriCare as they are not accepting any new providers in eastern North Carolina.
Currently, we are considered “out-of-network” with other insurance providers. We are happy to accept all patients - it is usually a similar co-pay as any local in-network provider.
We print a document called a “superbill,” for you to submit to your insurance for reimbursement.
NOTE: Unfortunately at this time we are unable accept new patients with primary insurance:
